How Is Aesthetic Smile Design Carried Out? A Step-by-Step Guide

September 9, 2026

Most people picture bright white teeth when they hear the term smile design. In reality, the clearest sign of good smile design is that it goes unnoticed: the person opposite you does not remark that you have had work done — they simply find your smile attractive. Here we set out the process we follow at our clinic in Eyüpsultan, Istanbul, step by step.

Step One: Analysis

The design begins with the face, not the teeth. At the first appointment we record facial and intraoral photographs together with video of your smile. We assess how far the lips part when you smile, how much gum the upper lip reveals, the relationship between the dental and facial midlines, and whether the smile curve follows the line of the lower lip.

The same proportions do not suit everyone. A defined, angular tooth form that looks right on a younger patient can appear artificial on an older one. Gender, facial proportions and personal expectations all shape the design directly.

Step Two: Health Assessment

Aesthetic work is always built on a healthy foundation. Decay, gum inflammation and teeth requiring root canal treatment are addressed before the design is executed. Porcelain placed against inflamed gum tissue quickly loses its marginal fit, and the aesthetic result deteriorates with it.

Grinding habits are assessed at this stage too. Porcelain restorations placed without a planned night guard carry a markedly higher risk of fracture, which makes habits a direct part of the treatment plan.

Step Three: Digital Planning and Trial

The records are transferred into design software and the proportions of the new teeth are planned. You see a simulation of the result on your own photograph and can request changes at this point.

Then comes the most important step: the mock-up trial. A temporary material is placed over the teeth without any preparation at all. You see the new smile in the mirror, experience how it looks while speaking and laughing, and can even live with it for a few days. This is the stage that leaves the final decision with you before anything irreversible takes place.

Step Four: Choosing the Right Treatment

Smile design is not one treatment but a combination selected according to need.

Where colour is the only issue, whitening alone may suffice and no tooth structure is touched at all. For problems of shape, size and mild irregularity, laminate veneers are preferred, usually requiring around half a millimetre of preparation and sometimes none. Where a tooth carries large fillings, has had root canal treatment or is deeply discoloured, a full-coverage zirconium crown is more appropriate.

When teeth are significantly misaligned, the most conservative approach is to reposition them first with orthodontics or clear aligners rather than reducing healthy tooth structure. Although this lengthens the process, it is usually the better decision for preserving natural teeth, and we recommend it openly to our patients.

Step Five: Placement and Review

After preparation, impressions are taken and you wear temporary restorations during the laboratory stage. When the permanent restorations arrive they are tried in first; once colour, form and bite are verified, they are bonded in place. A review appointment follows within a few days.

How Long Do Results Last?

Porcelain restorations resist staining well and, with good care, serve for ten to fifteen years and often longer. Longevity depends on daily oral hygiene, six-monthly reviews, avoiding biting hard objects and wearing a night guard where indicated.

Because unrestored natural teeth change colour over time, regular professional cleaning helps keep everything in harmony.
